Two infrastructure signals matter most for serious work-and-life: healthcare you don't have to fly out for, and internet that doesn't bottleneck your day. This list takes the intersection — cities where medical care is rated at international tier (English-speaking staff, JCI or equivalent accreditation) AND typical residential internet clears 200 Mbps. It's a tighter set than either filter alone, and it's where multi-year nomad bases tend to converge: tier-1 capitals, medical-tourism hubs, the established East Asian and Middle Eastern picks.
